If you would like to invite the Lieutenant Governor to present a message at an event, please submit a request through the Event Request form, in addition to submitting this form.
Types of Commemorative Messages
- Certificates recognize outstanding achievements by Hawaiʻi residents.
- Special Messages mark significant occasions and public events in Hawaiʻi, such as conventions, festivals and banquets; electronic copy also available for souvenir programs.
If you’d like to request a Proclamation or Military retirement message, please contact the Governor’s office.
Guidelines for Commemorative Messages
Due to the volume of requests received, please observe the following guidelines.
- All requests must be submitted using the form below, at least two (2) weeks prior to the requested completion date.
- All requests must be submitted by an individual or organization based in Hawai‘i, preferably submitted by an official representative of the local entity.
- The Office of the Lieutenant Governor reserves the right to accept or decline any request, regardless of whether a similar request was accommodated in previous years. We also reserve the right to edit submitted text for length, accuracy, etc.
- All issued commemorative messages are to be used for a single event or recognition and are not to be used for profit or commercial advertising. The text of the issued document must not be altered in any way without the consent of the Office of the Lieutenant Governor.
